Ok well im still confused because now i have coflicting answers to this question. I was the opponent in question during this match. I asked my judge SLVRSR4 this same question yesterday and i was told the official wizkids rules say action tokens placed on a character dont actually go into effect until the end of your turn. So you would only be able the use the Hellfire Chain ability when you begin your turn with 2 action tokens already in place because placing and clearing action tokens are the very last thing you do. This makes sense to me. I don't want to seem like im crying about it but id like to know for future reference.
You never asked me about this. I heard you talking to Chris about this. Action tokens get placed on a character at the end of their ACTION. Ghost Rider would be able to do it on the turn that he recieved his second action token, and then once on his turn before he would clear his action tokens.

Keep in mind....it is 2 unavoidable clicks. Even if your opponent rushes to KO Ghost Rider to avoid the 2nd free action at the beginning of the next turn, they will still take the Mystics damage. So, no matter what happens, your opponent will take 2 unavoidable damage for simply taking that second action. The only real defense against it is to make sure you are far away from Ghost Rider as it only works within 3 squares.
