Basically, to Spider-Man, there's a wall between him and Ben which makes them non-adjacent. So for instance, if Spider-Man had support he would not be able to use it on Ben, and if they had been opposing characters, Spider-Man would not have to roll to break away from him.
Ben, however, does *not* see a wall, and thus considers himself adjacent to Spider-Man meaning that he *would* need to break away from him if he was opposing and could target him with support.
Another way to look at it is to imagine if either of the two had poison (and were opposing). If Spider-Man, in the ring, had poison and Ben was opposing he would *not* take damage at the start of Spidey's turn because Spider-Man sees a wall between them which prevents Poison from going off.
However, if Ben had poison, Spider-Man *would* take the damage at the start of Ben's turn since he doesn't see a wall and thus considers them perfectly adjacent (note that if Fair Fight was showing on the Event Dial Ben would then take a token after resolving Poison as he was just given an action that affected a Wrestler).
All clear now?

How unusual. I have updated Ben because under that set of rules his trait won't activate. Cheers for that.

(Blue-circle) Fair Fight: If any character that isnt a wrestler is given an action that would affect a wrestler, affect any square inside the Ring, or move the character into or through any square of the Ring, give the character an extra action token after the action resolves
Wouldn't this apply to Black Panther perplexing Spider-Man's defense?

Yes, very much so. Even a free action like Perplex will give the perplexer a token after it resolves if he targets a wrestler (friendly or opposing) with it
