I liked Danny as GR for most of the series. And tho I wasn't a big fan of Johnny in his own original GR series, I did like him in the Ketch GR books, up till he was made to look like Kano from Mortal Kombat.
And when Mackie left, the book just went downhill for me. When Mackie brought GR back for that one issue of Spetacular Spider-Man, I thought they could then correct some of the awfulness that had been in the now-canceled Ketch book, but nothing ever came of it. Sad, really.

That issue of Spectacular was awesome. I didn't mind the Noble Kale stuff myself, as it lended itself to being easily retconned further (see the current Heaven stuff)

Name the two characters who, in normal continuity, have killed the Dan Ketch Ghost Rider.

Half right.

Hmm... well, Blackout tore out a chunk of Danny's neck, and he drifted towards death whilst GR took over the body. He was brought back by Lilith as one of her Lilian for a few pages, in monsterous form, then tossed back. Dr Strange then healed Danny witht he help of some surgeon. Technically, he didn't actually die I guess.
Then there was the time Blade killed him him when Blade had all that power from the Darkhold.
Later, GR's entire body, with Dan in it, was swallowed up and absorbed by Zarathos, thereby dying. GR was then resurrected thanks in part to the efforts of Hellgate.

Ok, I'll go with Blade and Zarathos as being the two who killed Danny.

Lilin Ketch fighting GR was awesome stuff.

The Zarathos/ Ghost Rider fight was epic, in part because Zarathos was so nonchalant about the whole thing.
