He would never roll for Impervious for Mystics damage. So he would never be able to remove tokens. He would KO.
This is actually incorrect. Just because the damage cannot be reduced or is unavoidable damage does not mean you do not get to roll for Impervious. You still roll, but generally that roll does nothing.
This is something to remember because of figures like Hulk here and the old DC75th White Lantern Superman. The same ruling applies to Shape Change vs Battle Fury (see DoFP Mystique).
I agree that it doesn't make sense that you would still roll the D6 for this power, but that is the official ruling and has been for a few years now.

What part of the word UNAVOIDABLE do people not understand, Hulk can't ignore it, counter it, reduce it, or super sense it. The Mystics damage take place AFTER actions resolve so Hulk checks to see how many tokens he has stockpiled, rolls for imperv, removes tokens to negate damage, and THEN once that is resolved Hulk takes the mystic damage. If you don't want Hulk taking Mystics damage play him with someone who prevents the use of Team Abilities. Or just find the Mystic on the opposing team and....wait for it.... KILL THE BLASTED THING.

And he does neither of those things. From the looks of it, he simply does not take the damage.
Again, this is not different than any of the many figures that does not take unavoidable damage when using Mind Control.

Impervious is the only one that you MUST roll a d6 regardless if it will have any effect... things like Unavoidable Damage, Penetrating, etc. will make the result of the roll moot, but you still should technically roll it every time the figure is dealt damage. SSenses and SC are optional and will help in rare circumstances when the result is ignored (eg. DoFP Mystique as mentioned), but it can be done.

A power can still be created that supersedes unavoidable. It's a typical rule in most games that text on the card will overrule a general statement in the rulebook.
The rules-lawyers will complain otherwise, but I think the intent of the power is pretty clear.
Hulk gets attacked and then rolls for Impervious. After the roll you figure out how much damage he's going to take. For each 1 damage he takes, you take off a Rage token instead. If he still is taking damage after removing all his Rage tokens, he's KO'd. If he doesn't, then he took no damage and doesn't get KO'd.

If Hulk rolls a critical miss while on the Raging Out click, would he remove one token and not be KO'd?
Yes because it works for damage taken regardless of source. Hulk is dealt 1 damage from the crit, rolls Imp (which does nothing), removes a Rage token, gains an action token, and the turn goes on.
