Has this been officially stated anywhere? Because RAW (Read as Written) it says put ONE token on when you'd take pushing damage. It doesn't say put on one per token (ie: Still just one token if exhaustion is in play)

It actually has nothing to do with the number of tokens as novablast does not add tokens, but rather the amount of pushing damage dealt. You add one token for every instance of pushing damage avoided (in the case of NB that would be 2). See this thread for the citation of the rules arb's statement and an entire discussion of this topic of this combo.
When you attack her to make her push just to get her down to 18 defense nope they'll put a token on enduranceand every body stays at 19 defense for another round.....

From the DC E&C Document, under "Endurance": One endurance token is placed on the card for each pushing damage dealt to the character.
