Let's say I have Dr. Strange (with his 18 Defend) surrounded by allies (Fig A and B). He uses his Perplex to increase the defense value of one of his teammates (Fig A) to 19, while Fig B has 16 defense.
An opponent attacks Fig A and B at the same time. Can I declare that, for figure A I don't want to use Defend, but for Fig B I do? What if the attack isn't simultaneous? Can I then declare it?
Defend is an optional power,an adjacent figure can choose to use the DV or not.You wouldn't be allowed to perplex up the the borrowed DV though(the character with the 16 DV can take the 18DV from Dr. Strange,but cannot have that further perplexed to a 19 DV)
Actually if the power is active then all adjacent figure would have the 18D regardles of their own defense.
So in the case if both figures were attacked at the same time then u would need to decied have them both at 18 or allow one to have his 19 and the other with 16.
Also if you choose to turn of defend to allow the figure to have his 19D then it is off for the turn. meaning if the 19D got tagged for damage and his D lowers you can not then use Defend to give him the 18D for the rest of that turn.

I don't see anything that implies that all adjacent figures must take the defense value or that you have to cancel defend to not use it. It says "can replace," which means that you can do it while the power is active, not that you must do it while the power is active.

This is correct.
It is the same as you have with the SPs and TAs saying that you "can use" X power.
