If I have Toll face up on the field and my opponent attacks twice with mataza the zappa, how much LP does it cost him?

You pay each time you declare an attack.

So:

TP declares attack [Mataza the Zapper] vs. X; pays 500LP because of [Toll]
(resolve attack)

TP declares attack [Mataza the Zapper] vs. X (2nd attack); pays 500LP because of [Toll]
(resolve attack)

i have that problem with kuriboh. it says,
"Kuriboh
(Metal Raiders, Starter Deck Yugi Evolution)
Discard this card from your hand. Make the Battle Damage inflicted to your Life Points by 1 of your opponent's monsters 0. You can only use this effect during your opponent's Battle Phase.
and yet, i've heard that it can only block one attack from mataza.
