For quick reference reads...

Ongoing: At the start of the recovery phase, put the top card of your deck into your KO'd pile.

Exhaust a Manhunter character you control, replace Plans Within Plans --> Return target card from your KO'd pile to your hand.

Need a quick ruling here...when I replace Plans Within Plans, and it goes into my KO'd pile as part of it's cost, on resolution of the effect, can I select PWP to return to my hand?

It is not in your ko pile when the card requires you to choose your target, therefore you can't choose it to return to hand.

You can choose it if you have another copy in the KO pile. You declare what you want to do before you pay your cost. (KOing the PwP)

When you replace Plans Within Plans is it from your hand or the top of your deck?

Replace is always from your deck. Reservist is from your hand.
