The following is an extract of the Dair VS Vincent Quaterfinals report:

Dair took the odd initiatives, and neither player had a turn 1 play. A turn 2 Maxwell Lord from Vincent hit Dair for 2 unopposed. Vincent had the initiative, but had no play. Dair recruited Sam Scudder ◊ Mirror Master, and beat into Vincent’s 2-drop.
Dair had the initiative on the fourth turn. He recruited Crimson Fox, and passed. Vincent recruited Dinah Laurel Lance ◊ Black Canary, “Pretty Bird” in front of Maxwell Lord. Before attacking, Dair teamed up with Gang-Up. His first attack sent Dair into Dinah. Vincent had Shake it Off, but Dair had a power-up. Dair had no further attacks, but played Death Trap on Vincent’s 4-drop. Amazingly, Vincent had a Death Trap of his own. The players laughed and shook hands. “That was nice.” The players were chatting and playing so fast at this point, I could barely keep up. Both of them were at a healthy 44 as the turn ended.
Diar lost the game because he lost his 4 drop. Now, what would you have done in his seat if given the above information??
Not sure ... could he have flipped his Team-Up while C. Fox's recruit was on the chain? This would have given him another JLI character (and thusly a Twin counter for Vivian), but I'm not sure of the legality of the timing.
It's hard to tell if you're saying there was a bad play, or a legitimate game error. My guess is that you're trying to say Crimson Fox wouldn't have been stunned, and so wouldn't have been Death Trap-able, if he had flipped Gang-Up sooner to team-up. But since Dair's Mirror Master isn't naturally a JLI character, and because Crimson Fox has the wording of a conditional trigger, there was no way that he could have flipped Gang-Up in time to put a counter on Crimson Fox. Her power never even triggered, due to the lack of another JLI character.
If you're saying there was an accidental initiative switch, on the other hand, then it does seem like there was a mistake, but I'm guessing that the reporter just meant to type that Vincent had chosen the odd initiative, since Dair won the previous game.
Crimson fox has a triggered effect that checks for a JLI character when she comes into play. With the trigger on the chain, he can flip a team-up and the trigger will resolve with another JLI character in play, allowing for the twin counter.
Well, you were right to be hesitant about the timing for Crimson Fox. :)
For the rules on Crimson Fox's conditional trigger, check out 502.5 in the Comp. Rules. There was no way for Dair to team his characters up in time; her power never triggered, so a Gang-Up wouldn't have mattered.

Actually the initiative was swapped even since turn 3. Could be a mistake from both players because Dair had no play on 2 and his opponent was attacking directly.

The error in this piece of coverage is in the first word; Vincent (and not Dair) took the odd initiatives. You can see that this is the case since Dair won the first game, giving Vincent choice of initiatives for the second game. I have fixed the error. As far as Crimson Fox is concerned, there is absolutely no way to time a team-up to allow her to receive a counter. The ability that gives her a counter never even triggers, as the condition for it triggering (control of another JLI character) is not met at the time Fox enters play. Both players were very aware of how the rules work, and very aware that Fox wouldn't be getting a twin counter. In fact, I believe Dair mentioned how unfortunate it was that this was the case. Anyway, I hope this clears everything up.
