In two-player games, do all 4 starting areas count for placing the object?

I wouldn't think so, no. Nobody is starting in those starting areas.

However, I know of no documentation that says one way or the other (probably because special objects weren't introduced until after the two player only maps became the more or less standard tournament size).

No... only the starting areas where players have actually set up.
