Only the initial target is injured, and EE only grants 1 SA regardless of the number of attacks. Best way to figure out the number of SA's something grants, or "Successful Attacks", is to count how many attack rolls you had to make for the attack, not including rerolls of any sorts. Duo Attacks and Flurry Grant 2, Pulse Wave and EE and Quake all grant 1.
