yes you leave the tokens overlapping, set the palnes however they fit. It is the token that counts as you will plot ranges and arcs and courses off of the token

I think if an Rule is not absolut clear speak with youre opoment and about it before the game begin and try to make the best. I woud say for costum made squads. The are one squad so they knew each other like other squads do

Yeah so long as everyone agrees to the House Rules and it isn't for an 'official' tournament, I don't think anybody'd have a problem with it.

and by official tournament they shoud be one who can answer the question befor it happen. Its ever better to make every thing clear before. In the Old CS was it this leader and wingman don´t have to check for collosion because they knew each other and use the same radio frequence. :)
