i also thought that the idea for a carnage effect could be done where you could place a card from your hand underneath him to give him a +1/+1 (not quite a counter, but not a power up either). this could be done twice per turn, maybe. if he got k.o.ed then all cards would go to your discard pile.
the 'drawback ' for him could be something like: your opponent may discard 2 cards from hand to place 1 card beneath him out of play.

That's basically the way I thought it could work, except I think she should be able to affect other characters as well, or at least have the option of doing so.
This seems possible for a Scarlet Witch character -
Pay 2 endurance -> Target opponent chooses odd or even. Reveal the top card of your deck. If that card's cost matches your opponent's guess, remove that many cards from the top of your deck from the game. If your opponent guesses incorrectly, reveal that many cards from the top of your deck and choose one, put it in your hand and put the rest on the bottom of your deck.

Pay 5 endurance -> Target plot twist which targets a single character now targets a different character of your choice. You may ignore combat role targeting restrictions. (You may retarget a defending character with a plot twist that targetrs an attacking character and vice versa.)

How would you be able to use this unless you already had one in play? Can you use a card's text while it's still in your hand? And if you needed one in play, why would you want to replace it with another card that has no ability once in play?
Here's some ideas for Scarlet Witch. Pay 3 endurance, characters with a cost of 3 or less are not unique this turn.
Pay X endurance and target an opponent and target X number of characters your opponent controls where X is the number of endurance paid. Your opponent must move those characters from one position to another.
Scarlet Witch comes in play with a Hex Counter. Activate, remove a hex counter, and pay one endurance, chose one. target player looses one recruit point this turn or you gain one recruit point this turn.
Cosmic: Activate: Search your deck for a plot twist. Reveal that plot twist and put in your hand. Shuffle your deck. You may not play that plot twist till the next turn. ( I figured she could be used to search for Pt's because it would be similar to her casting spells. The reason behind the delay is because it would take a turn to "cast" the spell.)

Did you simply not read the character text for scarlet witch proposed by tchalla??
It clearly states the following:

In regards to using a card's text if said card is not in play:
Yes, you can do so if the card reads as such. For instance, Infinity Man<>Drax
The card states:
Reveal Infinity Man, return two ready characters you control with the version Forever People to owner's hands-->Character cards named Infinity Man cost you 1 less to recruit this turn. Use this power only while Infinity Man is in your hand.
Okay, so maybe tchalla strayed off the path of originality, but his idea was in fact playable.

There are (at lease) 3 cards that have "from hand" powers:
Inifinity Man
Scarlet Spider
Harley Quinn
Those date back to DC Origins, and the first two are common cards.
There are abilities that can be used from the hand, without one on the field, and have been for a while, so the proposed power would work.
