The first was the Ultimates issue 3 or 4 when it is stated that people were killed during Hulk's rampage.
Since this is not 606, canon was not changed.
It has since moved to 606 with the Illumitrati series and it was a retcon. So introducing it as a major change event, it was snuck in with a talking head that it has been happening for a while.
The reason I know it was a retcon and not an always event is because of Phoenix. It was never in Clairemont's storyline for Jean to die in judgement from the Shiar but Marvel Editor-in-Chief Jim Shooter demanded it because "Heroes don't kill so Jean has to pay for it".
I'm with you Spiderhulk, but I blame Bendis. So until he goes back to playing with just his little corner of the Marvel Universe, I am not buying.

Don't worry, it's not like they're letting 616 Hulk eat people now, it's just a retcon that is more believable in my opinion and it's not really something that they did for shock value...
If you want, just replace 'responsible for many deaths' with 'could be responsible for many deaths in the future' as his rampages can really lead to civilians dying (imagine him going Hulk smash in a major city)...

It wasn't in his story for it NOT to be Jean, but instead have Jean hiding in the ocean.

Hmmmm... If the Hulk started eating people I'd actually buy his comic.
