I've never heard any urban legends about 'Taker. It was always Kane that I heard people thinking had different people under the mask (nope)
DOINK, on the other hand, has had at least three people under the makeup.
I think the Undertaker one mainly stems from the horrible "fake Undertaker" angle after the Royal Rumble that eventually led to the Undertaker vs. Undertaker match, but I'm not entirely sure.
But yeah, The (real) Undertaker has always been Mark Calloway

Well that and the horribly aborted angle where undertaker was going to be revealed as an alien Bounty Hunter (which i think was a tie in to a movie)

Heh, Taker has always been Mark Calaway. Take a look at his face in 1990, the only thing that's changed are some more wrinkles some tattooes, and a scar from where Mable broke his orbital socket, thus he had to wear that silly mask.
A friend of his played the Underfaker, Dibiase's claim to buying the Undertaker.
Kane has always been one guy as well. Although that guy was once a killer Christmas tree, a Demented dentist, and a fake Diesel.
I only know of two guys who wore the Doink get up. Matt Bourne (Evil Doink) and Steve Lombardi a.k.a. The Brooklyn Brawler. The third guy's name escapes me.
