When does Replace then modify apply and when does it not?
Always. It never 'does [sic] not' apply.
What confuses the issue is that is that if something is modified before the value is replaced, the entire old value (including any modifiers on the old value) is replaced. Modifiers applied at the same time as the replacement (or after) will modify the replacement value.
So what you really want to know is when does the modifier modify the old value and when does it modify the replacement value.

This statement is actually no different than the one immediately above. Check the timing on th ICWO card; the modifier is applied immediately when the action is assigned to the character, but before anything actually happens. This is why you can use ICWO to increase, say, Range and be able to shoot a character that is technically too far away (and you wouldn't normally be able to perform the action).

Depends on the feat and TA description. Some occur when the action is given, like ICWO, other occur actually during something (attack, movement, etc). If it occurs when the action is given, see above. If it occurs during something, the it occurs at the same time (or even after) the replacement value. Timing is key.

Again, it depends. With those two specific TAs you mention, HYDRA occurs during the attack roll, so you'd replace the AV with Sinister Syndicate and bump up the ranged AV with HYDRA at the same time.

See above. The one this to look out for here is several powers (EE, Incap, PW off the top of my head) say that the damage value 'becomes' something until the end of the action. Since it specifies 'end of action', no matter what you do that damage value can't change until the action is over or unless something completely alters the way the power functions (Stunning Blow, Nova Blast).

Doesn't really matter what I can think of... what matters is what the game designers will come up with for new feats, powers, BFCs, etc. Rather than trying to make a long, boring, exhaustive list that would have to be constantly updated with each new batch of feats and such, just remember those basic timing rules. Now you are ready for any replacement value and modifier combo.
