When you revert/ alter ego/ bring in a character that is not part of your initial force i have a few questions about this:
1) If the tournament format is modern age and the possible reverted character is golden age can it still be used as the original character was legal at the start?
2) For team sheets in tournament play do these possible reverts/ alter ego's /character from outside the game (e.g. Galactus herald) need to be written on the sheet?

No. Modern is highly restrictive. If its not Modern Legal its restricted from Modern play.
That is why Modern sucks.

Yes. Your team sheet is everything you are using for that tournament. If its not written down you cannot / should not be using it.

Visible Dials and Pushing Damage need to be optional. This is the way.

We don't typically use build sheets where I play. But I still always state what characters (if any) I have on my sideline at the time I reveal my force to my opponent.
I'm not sure why you think that not filling out a paper means one would get to use one's whole collection. That's like saying that because we don't use build sheets, I can change my entire team in between rounds.

Note there are mechanics that bring characters into the game that are not part of your sideline and do not need to be written on Tournament sheets. Examples: Zuvembies, Hydra, Master Mold Sentinels, Madri, etc...

This is not how it works. You would bring Thor 202 in on his orange starting line (click 7)
Per the power on Donald Blake:
ALTER EGO:THOR, GOD OF LIGHTNING: Give Donald Blake a power action that deals no pushing damage and replace this character with CW #038 Thor, CW #202 Thor, or FF:CW #002 Thor on its orange starting line. After actions resolve, even if this power is lost, deal 1 damage to each adjacent opposing character. This power can't be countered.

Note that in this specific case CW202 Thor will use his orange starting line which is worth 33 points. So It's perfectly fine.

Well, first, the Thor would come in as an AE and on the appropriate starting line as well as only be worth the amount of points for the AE. So that example doesn't work.
There are also examples of starting a game with a force that is within the build total, and ending with a force that is beyond the build total. Team dials is a good example. A team that includes Zuvembies. Master Mold. Those are just a few.
