What if player 1 has a face-up monster which is connected to Call of the Haunted and a set Book of Moon, then player 2 plays Heavystorm on his turn, and then Player chains BOM to put the face-up monster face-down?...
Will the monster still be destroyed?
it states on Call of the Haunted "Select 1 monster from your Graveyard and Special Summon it in face-up Attack Position. When this card is destroyed or removed from the field the summoned monster is destroyed . If the summoned monster is destroyed, this card is also destroyed."
Is the monster still connected to CotH, since it's a countious trap and not a equip card(Premature Burial). Other trap cards are still connected to a face-down monster such as Spell Binding Circle.
Also what if the monster connected to Call of the Haunted was removed from play?..will Call of the Haunted still be on the field since the monster was never destroyed but removed.
Since the monster is turned face down it is no longer effected by Call of the Haunted, and would not be destroyed if Call of the Haunted is destroyed. That is of course unless something was not going to destroy Call of the Haunted being chained to Book of Moon, like MST or Dust Tornado.
So, if the chain is just heavy storm then BOM, and nothing else destroys COTH in that chain, the monster lives.
If something else is used to destroy COTH before BOM resolves, the monster dies.
The reason is because COTH only destroys the monster if it is face up. If a monster is turned face down, it is no longer connected to Call of the Haunted. This is different from Spellbinding Circle. Spellbinding Circle does not care about the postion of a monster, Call of the Haunted does. When COTH is used to revive something, it is always face up. Spell binding circle can be used on a monster of any postion, big difference.
This also applies to equips cards. When a monster is turned face down, or removed from the field, they are no longer effect by the equip card. Under normal circumstances any card "equiped" to a monster is normally destroyed. COTH is unique in that it is only destroyed when the monster it is equiped to is destroyed. Or when COTH is destroyed or removed from the field(giant trunade), it then destroys the monster it was targetting if the monster is still face up when the chain resolves.
Also, if the monster is removed form play then call of the haunted would sit on the field useless, and taking up space in your s/t zone.

But if you need assurance, here it is:
Call of the Haunted needs to be destroyed while it is targeting a face-up monster. Book of Moon, IMT, etc, will sever the monster from Call and make Call stay on the field doing absolutely nothing but taking up space in your S/T zones.
THe same applies to when your monster is anything but "destroyed". For example, if your opponent's BLS-EotB or Dark Core removes the monster from play, Call will remain on the field because the monster wasn't "destroyed". It was removed from play. If the monster is used as a tribute (for any reason whatsoever), the Call stays on the field doing nothing as well.

Yes, the monster must be "destroyed" in order for Call of the Haunted to leave the m/t zone (aside from the usual m/t removal)
So if it's
Tributed
Removed from Play
Flipped face down
It will not be destroyed and just sit in your m/t zone dead.
And DO NOT say "..but it gets destroyed in the Game Boy game anyways," since the Game Boy rules were VERY outdated the day they were released because rules are constantly updated and cards being errata'ed and such while the game is being developed and made, which is why World Championship 2004 was missing half of LOD and half of PGD, despite our card sets being up through DCR at the time, so people had NO reason to get all mad and upset about missing cards or weird rules that don't match the current gameplay.
