If I name Mogo and reveal the location, can I take it?

Yes, as you name the name of a character and find a card with that name.

So, as a follow up that I think I know the answer to...

If you recruit Katana the character (Kc) while you have a Katana the equipment (Ke) equipped to a character, I'm guessing it KO's the equipment as Mogo the character KO's Mogo the Location and vice versa, but equipping Ke while you have Kc out would not result in anything as Ke is not unique.

And I can't seem to think of anyone else.

That Mogo situation hasn't been a problem since MHG, actually, when the uniqueness rules were adjusted. You only have to get rid of something if both the new object and the old one are unique.

Both have to be unique? So the rules were changed around MHG, but wouldn't Mogo still be a problem as both are unique, unless there is some text I am missing here.

Yeah, Mogo and Mogo still get in each other's ways. Whereas Katana and Katana get along great.

Yes, Katana may wield a Katana, but Mogo can't orbit Mogo. Funny how that works... ^_^
