The attacker then rolls 2d6. Only one attack roll is made, regardless of the number of targets of the attack
Effects that allow dice to be rerolled or replaced are used during this step. Reroll effect must be used first, followed by effects that replace die, and then finally apply any effects that otherwise alter the attack roll (such as effects that increase or decrease the result).

It really does go Rerolls -> Replacements -> Finalized.

If you had decided you wanted to use PC to reroll it, then the attack wouldn't have progressed to the Die Replacement part yet. Every effect that could reroll the attack gets a chance to happen first before any Die Replacements get a chance.

They could absolutely replace dice that have already been replaced, but the active player gets to choose what order those replacement effects resolve. If you're the active player, I would suggest probably making your opponent's replacement effects happen before yours. If they pass on using them, then they've resolved and they can't change their mind after you use your replacements.
It's still once per attack, since the ability triggers off of an attack roll. It triggers once, resolves once, and wouldn't trigger again unless another roll was made.
