1. Can the Avengers/JLA TA be used to activate phasing?

No. Phasing/Teleport is a power action. The "free move TAs" all function with move actions only.

No. Mystics only activates when the character is dealt damage from an attack. In this case, the Mystic is not dealt damage from an attack; it is dealt damage from Master Mind.
(and, no, the Master Mind character doesn't take the Mystics click, either. It's a total wash; no retaliation.)

No. The capture rules for characters like Sentinel and Sinestro specifically say that the ability may not be used with the multi-attack. They've got one shot per turn, and one shot only.
(and if this is causing you confusion based on the new Advanced Sentinel rules, check the Marvel E&C. There is an errata entry addressing this specifically.)

If you must reroll one of the pair, then you (usually) reroll both. Only real exception to this rule is the Lucky Break feat (only a single die is rerolled).
By the way, what exactly do you mean by "field of play"? The dice don't necessarily have to stay on the map itself. As long as they stay on the table, that's fine. Hitting the floor is technically a no-no, but short of that, I'd let your nephew slide. If he's really bad at it, get a box with relatively deep sides (maybe the lid to an old board game) and have him roll the dice inside the box.

If they can draw a clear LoF to the target and there is no restriction on making a RC attack against that target for other reasons, then yes.:disappoin However, under most circumstances, figures block LoF, though there are several situations where that is not the case. LoF between a grounded figure and a figure on the rim of elevated terrain is no blocked by grounded figures. If the attacker is using Trick Shot or Pulse Wave, the LoF is also not blocked by figures; the same is true if the target is assigned Coordination and the Stealthy figure has the same Team Symbol. Also, when the target is soaring and none of the figures between it and the attacker are soaring. The target is a Giant or Colossal figure and none of the figures between it and the attacker are Giant or Colossal.
There may be a few others (and if there are I'm sure someone will post them) but these are what I come up with off the top of my head.

Incap...yes since the base damage is 0.*
MC...no since there is no damage.
There's a difference between "0 damage" and "no damage" for this reason.
(And a critical miss on an MC does no damage to you.)
* - Nothing sucks more than trying to Incap a Mystic and rolling a Critical Hit.
