When you go to push and place a token on Endurance, do you STILL get a second action token, or is the one palced on the card IN LIEU of an action token?
The reason I ask is a) can you theoretically just act every turn? b) If an Incapper or 2 hits you, can they keep triggering Endurace (of course, you could just choose to take the normal action....so that settles that). But a) is still a valid point...

Choose a character.
When the character would be dealt pushing damage, you can instead choose to deal no damage to the character and put an endurance token on this card. If there are endurance tokens on this card when the character is given an action, after the action resolves, roll a d6. On a result of 1 or 2, deal the character unavoidable damage equal to the result plus the number of tokens on this card, then remove all tokens from this card.
From the description, we can see that Endurance has absolutely nothing to do with action tokens; it is all about the pushing damage.
So you place action tokens normally and follow the normal rules for number of consecutive actions (meaning, you can't give a character a non-free action if it already has two tokens).

...except that you have to roll to see for damage(sic) for every single action you take after you've placed at least one token on Endurance, so you'd have to roll after your action on turn 4 too.

But in my example, you've passed on turn 3, so on turn 4, you'd be placing the only token on the guy...he wouldn't be pushing so the card gets nothing, right?

The card says each action. So, it's each and every action.

The card gets no extra endurace tokens, correct. However, the roll to determine if the character takes unavoidable damage occurs whenever the character is given an action (except a free action) if there are already tokens on the card.
So any action by that character after the first push (and placement of the first endurance token) will result in a roll.

Edit Just realized it might help to post the part of the DC E&C that says that free actions don't trigger a roll. Otherwise, a literal interpretation of the original description would suggest to the contrary.

Doesn't matter if you're pushing or not, once there's at least one token on the card, you have to roll after each and every non-free action you take, no matter if you're pushing or not.
You only put tokens on the card when you would take pushing damage, but you still have to roll for damage after every non-free action you take until you get rid of the tokens.

Thanks all (especially NormalView....I owe you a beer for every time you've helped me!).
this brings the card more into balance...the way we played, it was the greatest 5 points you could play this side of 5/8's of a Protected!
So basically, once you decide to psuh and give it to Endurance, you're adding a toekn to End every non-free action you take, regardless of push or not.
And again, you could choose to push THE FIRST TIME and not use Endurance and simply push normally. The 'piling of toekns onto Endurance' starts once you decide to skip pushing damage via the Feat, right?
