So, it seems alot of people (especially new people) get all confused about the chain. I thought maybe it would be good for people to have a visual representation of how things go, so I made a picture. I hope I didn't screw it up and I hope it helps some people. If people have a question, refer them to the picture (assuming it is correct and makes sense to anyone else besides me). Here is the pic: http://img39.exs.cx/my.php?loc=img39&image=vs_chain.gif
I do have a suggestion, add a circle labeled "Unresolved Effect?" With "Yes" taking you to the Effect resolution circle and no taking you to the end phase/step/substep.
So the "Unresolved Effect" circile would be where the "Effect resolution" circle is now and the "Effect Resoltion" would be to the left.
cdaniel, it seems like that would be redundant since the arrows coming from the resolve effects state tell you that one way is for an empty chain and one is if there are still effects. I can try and add it later if that would make it easier for people to understand.

Hi Thing 11,
I think I can see where Chad's coming from. You shouldn't be able to hit the circle "Effect Resolution" if the chain is empty. An alternative suggestion would be to rename that circle to "Both players have passed in succession" and then allow the existing arrows to take care of resolution.
When you go from Player B passing back to Player A (after player B has played effects), if player A passes at this time, it goes to effect resolution, not back to Player B, right? (both passed in succession)
Any visual aide that helps people understnad the chain, passing priority and resolving effects IMO is a good thing. If this chart helps even a few people to get a better grasp of how to properly resolve a chain then it is well worth it.
