Forms I believe can be taken up at the event so there is no need to fax it in advanced (In the Players Packet you'll notice that they mention that release forms will be available at the event, which is why I don't see any reason they'd require them to be in before the day of registration for the event).
As far as forms for the LCQ's go, you do not need a form to my knowledge to play the LCQ's, it's basically like playing any other normal PCQ except it's a Last Chance Qualifier instead.
Prizes can be found at the UDE PC NY website I'm pretty sure, but I wouldn't imagine it'd be much different from the PCQ prize structure, except I have heard that LCQ's do not offer $250 to the winner (since I haven't had the chance to ever attend a PC, etc. I wouldn't know for sure).
Hope that helps, anyone else with more insight and information can definitely add as I know my information is a little sketchy.

From what I remember from SoCal, I managed to stumble my way through 2 gruling rounds of the LCQ and won one of the spots. The next day I registered for the PC which included filling out a few forms, them taking a picture (to prevent fraud I assume) and giving a blood and DNA sample. Very uncomfortable. The guys at the booth had really cold hands...
