Correct and it doesn't matter if it is from an attack or something like Poison or the Mystics TA.

I think it matters, since it says "close attack."

Hitting with a close attack is what gives the Bleeding token but after that the Bleeding token increases all damage dealt to that character.

Yep, they errata'd the card to indicate that he has Flight.

Why does he have Gotham City and not GC Underworld?

I would say because GC Underworld is for the organized crime element of Gotham. something Man-Bat wouldn't be part of as he is more a victim of his condition which makes him a menace more than criminal
